By participating as a driver-member of Fare Co-op, I hereby grant my express, informed, and continuing written authorization to Hub Cyber Security Ltd. and its affiliates (collectively “Hub”), acting on behalf of Fare Co-op, to collect, process, use, disclose, and retain my personal information for the purpose of conducting initial and periodic background screenings to determine and maintain my eligibility for participation in the SecureRide™ program.
This authorization includes, without limitation, the procurement and review on a recurring basis of consumer reports and investigative consumer reports, including motor vehicle records (MVR), driving history, criminal history, identity verification data, and any other records deemed reasonably necessary for eligibility, safety, and compliance purposes. I acknowledge and agree that such reports will contain personal information relating to me.
I understand and agree that:
- This authorization is a mandatory condition of my participation in the SecureRide™ program and of my status as a driver-member of Fare Co-op;
- Hub and Fare Co-op may obtain such reports on an ongoing and periodic basis throughout the duration of my participation, without the need for additional consent each time;
- The personal information collected and the resulting reports may be used, shared, and retained by Hub and Fare Co-op strictly for program administration, safety, compliance, and eligibility determinations, in accordance with applicable law.
- The personal information collected and processed pursuant to this authorization will be handled in accordance with applicable privacy laws. I understand that, subject to statutory limitations and exemptions applicable to background screening and safety compliance, I may have rights to request access to, correction of, or deletion of certain personal information, and to receive information about the categories of personal information collected, the purposes for which it is used, and the categories of third parties with whom it is shared. I further understand that personal information collected for background screening, eligibility, fraud prevention, and safety purposes may be retained as required by law and may be exempt from certain deletion rights.
I may withdraw this authorization at any time by providing written notice; however, I acknowledge and agree that any such withdrawal will render me ineligible to participate in the SecureRide™ program and may result in suspension or termination of my driver-member status.
By signing below, I confirm that I have read and understood this authorization, that I consent to the collection and use of my personal information as described above, and that this authorization shall remain valid for the entire period of my participation unless and until revoked in writing.
